Death certificates are available from 2009 – present for deaths that occurred in Florida from the Franklin County Health Department. Deaths prior to 2009, in Florida, may be requested by contacting the Vital Statistics State Office at 904-359-6900 x 9000 or by visiting https://www.floridahealth.gov/certificates/certificates/
Only those individuals who are authorized (direct relative, funeral home, beneficiary) may receive a death certificate listing the cause of death. Picture identification is required.
If the death did not occur in Florida, please follow the link to request death certificates from other sates: https://www.cdc.gov/nchs/w2w/
Our hours of operation:
Monday thru Friday, 8:00 A.M. to 4:30 P.M. except holidays in Apalachicola (850-653-2111)
