Death Certificates

Death certificates are available from 2009 – present for deaths that occurred in Florida from the Franklin County Health Department. Deaths prior to 2009, in Florida, may be requested by contacting the Vital Statistics State Office at 904-359-6900 x 9000 or by visiting http://www.floridahealth.gov/certificates/certificates/

Only those individuals who are authorized (direct relative, funeral home, beneficiary) may receive a death certificate listing the cause of death. Picture identification is required.
