We work on a variety of health projects developed and implemented by a local group called the Health Hub of Gulf and Franklin. The Health Hub collects and analyzes data to educate and mobilize communities, develop priorities, gather resources, and plan and implement actions to improve public health. The two documents used to do this successfully are the community health assessment (CHA) and community health improvement plan (CHIP).

Community Health Improvement Plan (CHIP)
CHIP can be used by health departments, as well as other government, education, or human service agencies, to coordinate efforts and target resources that promote health.
Working together, community partners have created goals and strategies to support the community’s top health priorities:
- Better Support and Access to Basic Services
- Awareness of and Access to Existng Services
- Crisis Care and Behavioral Health Services

Community Health Assessment (CHA)
A comprehensive community health assessment provides the general public and policy leaders with information on the health status of the population and existing assets and resources to address health issues. This health assessment provides the basis for development of a Community Health Improvement Plan.
