Birth certificates are available from 1917 – present for births in the state of Florida at the Franklin County Health Department and may be requested by contacting the Vital Statistics State Office at 904-359-6900 extension 9000 or by visiting FloridaHealth.gov.
Only those individuals named on a Florida birth certificate, parent or legal guardian may receive a copy of the document. Picture identification is required.
